Admitted to Practice New York, 2016 Education State University of New York, Buffalo School of Law J.D., 2015 State University of New York at Potsdam B.A., cum laude, 2011 Ms. Hazen focuses her practice on workers’ compensation. She represents insurance carriers and self-insured employers in a wide range of workers’ compensation claims. Ms. Hazen handles all aspects of litigation, including initial contact with employers during preliminary investigation, filings, witness preparation, routine hearings, lay testimony, medical depositions, drafting of Memorandum of Law, and summations. Ms. Hazen also prepares and drafts Applications for Board and Full Board Review, as well as Rebuttals to the same. Ms. Hazen negotiates and drafts final settlement agreements to end liability, both medical and indemnity, frequently encompassing Medicare set aside, and third party settlement issues. In addition, Ms. Hazen devotes some of her practice to the interplay between workers’ compensation and Medicare, focusing on Conditional Payment issues. Ms. Hazen has developed a particular focus on the Prior Authorization Request (PAR) process for medical treatment and services. She has presented on various aspects of this topic at seminars and virtually for clients. Ms. Hazen is a member of the Erie County Bar Association.
